What is ferritin?

Raising a low ferritin starts with knowing what the number tracks: ferritin is the storage protein that banks iron for later, so your blood ferritin level is the clearest proxy for your total body iron stores. When ferritin runs low, it is telling you the reserves are running down. That one number reflects the iron sitting in the bank, not just what is circulating today.

Ferritin also gives you an early warning. Because ferritin tracks stored iron, a falling ferritin is often the first lab sign that reserves are being depleted, well before hemoglobin drops or a standard blood count looks off. By the time anemia shows up, the stores have usually been empty for a while.

Normal vs. optimal ferritin ranges

Here is the catch with ferritin: the "normal" range printed on a lab report is wide, and the low end many labs use can quietly miss iron deficiency. A result sitting inside the reference band is not the same as an optimal result. Those ranges were largely built from historical populations that included people who were themselves iron-deficient, and lower cutoffs, often under 30 µg/L, do a better job of catching a real shortfall. Reference ranges describe where most people land, not where you feel your best, which is why a level worth a second look can sit inside the lower part of the "normal" band rather than below it.

The exact deficiency cutoff moves around. Iron deficiency is commonly flagged somewhere below roughly 15 to 30 µg/L, and the threshold shifts with the situation, including pregnancy, where a higher floor is often used. The number alone tells you little without that context.

Signs your ferritin may be low

You can feel low ferritin before it ever becomes anemia, so a normal hemoglobin or blood count does not let iron off the hook. In one trial, iron supplementation eased fatigue in non-anemic women with ferritin under 50 µg/L, a reminder that symptoms can arrive while the blood count still reads fine.

The symptoms tend to be vague on their own and telling as a cluster. Common signs of low iron stores include:

  • Persistent fatigue and low energy
  • General weakness
  • Pale skin
  • Shortness of breath
  • Headache
  • Dizziness or lightheadedness

There is one sign worth calling out on its own. Diffuse hair shedding, known as telogen effluvium, has been associated with low serum ferritin in women, and separate work found lower ferritin among people with the condition. This is an association, not proof that low iron causes the shedding, but it is a reason to check.

What causes low ferritin

Low ferritin comes down to simple math: when the iron leaving your body or the demand for it outpaces what you take in and absorb, the reserves shrink. The usual culprits fall into a few recognized categories, from inadequate intake to blood loss to malabsorption, sometimes with inflammation working against you behind the scenes. Each one plays out a little differently.

Low dietary intake

Diets that run short on iron are a common driver, especially patterns without much heme iron from animal foods, as with some vegetarian and vegan eating. Plant-based, non-heme iron is absorbed less efficiently, so even a diet that looks iron-rich on paper can fall short in practice.

Blood loss

Chronic or heavy blood loss depletes iron directly, which makes it a leading cause. Heavy menstrual periods and slow gastrointestinal bleeding both warrant investigation, particularly when the loss has no obvious explanation.

Poor absorption

Sometimes intake is fine and the gut is the problem. Celiac disease, low stomach acid, and gut inflammation can all blunt absorption, and during inflammation the hormone hepcidin rises and blocks iron uptake, dragging ferritin down despite a reasonable diet.

How to increase ferritin levels

The core approach is straightforward: eat more iron-rich foods with an emphasis on well-absorbed heme iron, pair plant iron with vitamin C, keep absorption blockers away from iron-rich meals, and add a supplement when diet alone cannot cover the gap. Then recheck your level to confirm the reserves are actually rebuilding. Iron stores respond slowly, so consistency beats intensity.

The sequence in practice:

  1. Eat iron-rich foods, prioritizing heme sources.
  2. Boost absorption by adding vitamin C.
  3. Keep tea, coffee, and calcium away from iron meals.
  4. Supplement if diet is not enough.
  5. Retest ferritin before you stop.

Eat more iron-rich foods (heme vs. non-heme)

Dietary iron comes in two forms, and they behave very differently. Heme iron from animal foods is absorbed efficiently, in the range of 15 to 35 percent, while non-heme plant iron sits near 2 to 20 percent and is far more sensitive to what else is on the plate. Leaning on heme sources gives you more usable iron per bite.

Iron content by food varies, but the pattern holds: organ meats and shellfish are dense heme sources, while legumes, tofu, fortified cereal, and greens carry plenty of non-heme iron worth pairing wisely.

Boost absorption with the right pairings

How you combine foods can matter as much as what you eat. Adding vitamin C to a plant-iron meal sharply raises how much non-heme iron you absorb and can offset inhibitors like phytate sitting in the same meal. It is one of the easiest levers to pull.

A few practical pairings that help:

  • Squeeze citrus or add strawberries, bell peppers, or tomatoes to a bean or lentil dish
  • Follow a plant-iron meal with a vitamin C-rich fruit
  • Eat a little heme iron alongside non-heme foods, which lifts uptake of the plant iron in the same meal

Avoid absorption blockers

Some foods and drinks quietly work against you when they share a plate with iron. Tea and coffee are the biggest offenders: their polyphenols, or tannins, can significantly cut how much iron you absorb. Phytates in whole grains and legumes and calcium from dairy or supplements pull in the same direction.

Common inhibitors to time away from iron meals:

  • Tea and coffee (tannins)
  • Calcium supplements and large dairy servings
  • High-phytate foods eaten without a vitamin C boost

The fix is timing, not elimination: separate your tea, coffee, and calcium supplements from iron-rich meals by an hour or two and you keep most of the iron.

Iron supplements: types, dosing, and timing

When food cannot close the gap, oral iron supplements are first-line, and the common forms, ferrous sulfate, fumarate, and gluconate, all work. How you take them shapes both how much you absorb and how your gut tolerates them.

The timing trick is counterintuitive. Taking iron every other day, or as a single morning dose, actually absorbs more per dose than splitting it across the day, because large or frequent doses spike hepcidin and blunt the next dose. Less can genuinely be more here.

Tolerability improves too. Alternate-day dosing reaches comparable repletion with fewer gastrointestinal side effects than daily dosing, which makes it easier to stay consistent over the months repletion takes.

One safety line worth respecting: without medical supervision, adults should not exceed the tolerable upper intake level of 45 mg per day of supplemental iron. More is not better, and a clinician can guide higher therapeutic doses when they are warranted.

When to consider IV iron

Intravenous iron enters the picture when oral iron falls short. It is typically considered when pills fail or are not tolerated, when absorption is impaired, when losses keep outpacing intake, or when rapid repletion is needed. This is a clinician-directed route, not something to arrange on your own.

IV iron is sometimes used for iron deficiency without anemia as well, though the payoff for symptoms is not guaranteed. In non-anemic iron-deficient adults, one trial found IV iron did not improve fatigue, so the decision is best made with a provider weighing your particular situation.

How long it takes to raise ferritin

Rebuilding iron stores is a slow project. Even after hemoglobin returns to normal, refilling ferritin with oral iron commonly takes about three to six months of continued treatment. The blood count recovers first; the reserves lag behind.

That lag has a practical consequence. Because stores trail the blood count, it pays to keep treatment going and continue past the point hemoglobin normalizes, then recheck ferritin, not just hemoglobin, before stopping. Otherwise you risk stopping with the tank still half empty.

How to test and monitor your levels

A serum ferritin blood test is the primary way to read your iron stores, and it is often ordered alongside additional iron studies, serum iron, transferrin saturation, and total iron-binding capacity, to interpret the number in context. One marker rarely tells the whole story.

There is a wrinkle worth knowing. Ferritin also climbs with inflammation, so a normal or even high ferritin during an illness does not always rule out low stores, because inflammation raises ferritin independent of iron. Context is what turns a number into an answer.

The sensible move is to retest periodically during treatment and to see a healthcare provider to pin down the underlying cause rather than self-treating indefinitely. Finding why iron ran low matters as much as topping it back up.

When ferritin is too high

More iron is not always the goal. Persistently high ferritin, especially alongside elevated transferrin saturation, can point toward iron overload and warrants evaluation rather than more supplements. High is a signal to investigate, not to double down.

Genetics often sit behind it. Hereditary hemochromatosis, an inherited HFE disorder, is the most common inherited cause of iron overload, and it is exactly why taking iron without confirming you need it is a bad idea. Testing first keeps you on the right side of the line.
